A Phase 1 environmental site inspection is a records and observation review. It looks at site history, past uses, and regulatory databases to flag recognized environmental concerns. A Phase 2 goes into the ground: soil borings, groundwater samples, and lab analysis to confirm whether a concern is real and how far it extends.

Usually no. A Phase 2 is triggered when a Phase 1 identifies a recognized environmental concern, when a lender or buyer requires confirmation, or when a regulator has already sent a notice about the site.
